GREETINGS AND SALUTATIONS. THIS COMMUNICATION IS ABOUT THE DEFECTIVE TORQUE CONVERTER IN THE TRANSMISSION THAT TOYOTA IS AWARE OF. I ACQUIRED THE VEHICLE AT ABOUT 33,000 MILES AND I NOTICED THAT IT HAD THIS PROBLEM OF A SLIGHT INTERMITTENT SHUDDER, WHAT SOME WOULD CALL A SHAKE OR VIBRATION, DURING ACCELERATION COMING FROM THE TRANSMISSION. IT DID NOT AFFECT THE PERFORMANCE OF THE VEHICLE SO I IGNORED IT MAINLY BECAUSE I DID NOT HAVE THE TIME TO ADDRESS IT. AS TIME WENT ON THE CONDITION BECAME MUCH MORE NOTICEABLE AND TOYOTA SENT ME A LETTER EXPLAINING THAT THEY WERE AWARE OF THE PROBLEM AFFECTING SOME OF THEIR VEHICLES AND THAT THEY WERE EXTENDING THE WARRANTY. I HAVE ATTACHED PHOTOS OF THIS LETTER WITH THIS COMMUNICATION. I FOUND OUT THAT THIS EXTENDED WARRANTY IS CONSIDERED A COURTESY AND CERTAINLY IT COVERED UP TO A CONSIDERABLE MILEAGE HOWEVER MY POSITION IS THAT BECAUSE THIS CONDITION EXISTED AT EARLY MILEAGE AND GREW CONSIDERABLY WORSE WITH EXTENDED MILEAGE AND TIME, THAT PERHAPS YOU MAY CONSIDER THAT TOYOTA SHOULD CORRECT THIS PROBLEM REGARDLESS OF MILEAGE OR TIME. MY VEHICLE IS PAST THE ALLOWABLE MILEAGE IN THE EXTENDED WARRANTY. I KNOW TOYOTA DOESN'T WANT TO HEAR THAT BUT PERHAPS THIS PROBLEM THAT THEY ARE AWARE OF IS FOUND IN A RELATIVELY SMALL NUMBER OF VEHICLES THAT IT MAY NOT CAUSE THEM MUCH GRIEF AT ALL. THANK YOU.

THE INCIDENT OCCURRED ON 11/13/2019 @ 8:30AM. I HAD STARTED MY 2014 TOYOTA RAV4, PULLED OUT FROM UNDER THE CARPORT APPRX. 25FT AND PUT THE VEHICLE IN PARK AND FULLY ENGAGED THE PARKING BRAKE I ALWAYS DO THIS IF I NEED TO LEAVE THE VEHICLE RUNNING FOR ANY REASON. IN THIS CASE I WAS HEATING THE INTERIOR AS IT WAS CHILLI. I WENT BACK INSIDE MY HOME TO GET THE ANIMAL CRATE THAT CONTAINED MY CAT WHO NEEDED TO GO TO THE VET. AS I WAS LOADING THE CRATE INTO THE FRONT SEAT THE VEHICLE BEGAN MOVING IN REVERSE. I WAS BETWEEN THE OPEN DOOR AND THE VEHICLE ITSELF AND AFTER FREAKING OUT A SECOND LOOKED TO SEE IF THE PARKING BRAKE HAD DISENGAGED. IT HAD NOT.AT THAT POINT I WAS ALONG FOR THE RIDE AND THE CAR DRUG ME TOWARD THE LAUNDRY ROOM AND I WENT UNDERNEATH THE OPEN DOOR HOLDING ONTO THE ARMREST. WHEN I HAD TO RELEASE MY TRIP I WAS LAID OUT ON MY BACK WITH MY ARMS AND HANDS RAISED. THE RIGHT FRONT TIRE CAME INTO CONTACT WITH FIRST MY LEFT ELBOW THEN WRIST THEN HAND WRENCHING MY LEFT SHOULDER AS IT DID SO. THIS ACCIDENT SHOULD NEVER HAVE OCCURRED. THE SAFETY MEASURES FAILED. I BOUGHT THE CAR IN OCTOBER 2017 AND HAD NO TROUBLE DOING THE EXACT SAME THING AS DESCRIBED MANY MANY TIMES. TOYOTA HAS BEEN NOTIFIED AND NOW NEEDS TO GO TO ANY LENGTHS NECESSARY TO MAKE SURE THIS DOES NOT HAPPEN TO ANYONE ELSE.

MY CAR IS ONLY 70,000 MILES AND FROM THE PAST I AM EXPERIENCING MISSED MISFIRE OF MY TRANSMISSION FROM 2ND TO 3RD GEAR. THE CAR IS DRIVEN DAILY, BUT BEING FAIRLY LESS NUMBER OF MILES THIS IS A VERY UNEXPECTED BEHAVIOR. I CHECKED WITH A CERTIFIED MECHANIC, WHO ASKED ME TO GET IT REBUILT FOR MORE THAN $3,000. BUT COULD NOT TELL ME THE REASON FOR THIS ISSUE. HE SAID LIKELY TO BE TOYOTA TRANSMISSION ISSUE.

THIS COMPLAINT INVOLVES SUDDEN ACCELERATION. IT OCCURRED TWICE, ONCE WHEN SHIFTING FROM DRIVE TO REVERSE AND AGAIN WHEN YOU GET SHIFTING FROM REVERSE TO DRIVE. THE CAR WAS AT A STANDSTILL BOTH TIMES. THE FIRST TIME WAS APRIL 2017. I WAS DRIVING ALONG A NEIGHBORHOOD STREET AT 20 MPH. WHEN I STOPPED AND PUT THE CAR IN REVERSE, IT SHOT OFF LIKE A ROCKET. I APPLIED THE BRAKES; THE CAR STARTED SPINNING AROUND. I PULLED THE EMERGENCY BRAKE AND THE CAR STOPPED. THE SECOND EVENT OCCURRED ON JANUARY 4, 2018. I WAS BACKING OUT OF A PARKING SPACE; I STOPPED SO I COULD RENTER THE SPACE. WHEN I SHIFTED FROM REVERSE TO DRIVE, THE CAR ROARED OFF AND BECAME AIRBORNE. IT FLEW OVER THE MEDIAN AND HIT THE CAR PARKED IN THE SPACE OPPOSITE. THE TIME BETWEEN WHEN IT ACCELERATED AND WHEN IT HIT THE OTHER CAR WAS SO FAST I DID NOT HAVE TIME TO APPLY THE BRAKES. IT FELT LIKE WE WERE FLYING AND THE CAR WAS IN TOTAL CONTROL. I FEEL THERE IS SOME FLAW IN THE ELECTRONIC SYSTEM CONTROLLING THE GEARS. NO POLICE REPORT WAS FILED BECAUSE IT OCCURRED ON PRIVATE PROPERTY. MY PASSENGER WAS SLIGHTLY INJURED - A BRUISED CLAVICLE, PROBABLY FROM THE SEATBELTS.

RECENTLY I WAS DRIVING MY 2014 RAV4 NEAR DOWNTOWN TAMPA ON ADAMO DRIVE AND WAS TAKING A LEFT TURN, AND SUDDENLY I HEARD SOME KIND OF NOISE AND FELT LIKE MY VEHICLE WENT DEAD WITHOUT ANY ELECTRICAL POWER AND IT STARTED REVERSING ON IT'S OWN AND SPUN TO RIGHT. FORTUNATELY SINCE I WAS TAKING A TURN I WAS HARDY AT A SPEED OF ABOUT 10 MPH AND HENCE MY FRONT RIGHT PASSENGER SIDE DOOR & HEAD LIGHT ETC. GOT DAMAGED AND THERE WAS A VERY MINIMAL DAMAGE TO THE FRONT FENDER ON THE DRIVER SIDE OF THE VEHICLE BEHIND OUR RAV4. FOR THE SAME REASON NONE OF US - EITHER MYSELF, MY FRONT CO-PASSENGER OR THE DRIVER OF THE VEHICLE BEHIND OUR RAV4 WERE PHYSICALLY HURT AND WERE VERY SAFE AFTER THIS INCIDENT. THE DRIVER OF THE VEHICLE BEHIND OUR RAV4 INFORMED THAT HE HEARD A KIND OF STRANGE SOUND AND SAW OUR RAV4 REVERSING & TURNING RIGHT, AND HE MOVED AWAY INTO THE RIGHT LANE. THE POLICE OFFICER, WHO HAS COME ALSO INFORMED THAT SHE HAS COME ACROSS THIS KIND OF INCIDENTS WITH TOYOTA VEHICLES RECENTLY, AND DID NOT GIVE ME CITATION AFTER REVIEWING MY INCIDENT FREE DRIVING RECORD AND ALSO SHE COULD NOT FIND REASON FOR THIS FREAK INCIDENT WHICH LOOKS MORE LIKE A MECHANICAL DEFECT OF MY RAV4, AND ADVISED TO TAKE TO TOYOTA DEALER FOR FURTHER INVESTIGATION & RESOLUTION. ON FURTHER RESEARCH WE FOUND THAT THERE IS A SERIOUS RECALL - "ELECTRICAL POWER ASSIST SYSTEM" (EPS) FOR 2014 RAV4 WHICH WAS IDENTIFIED & DOCUMENTED BY TOYOTA ON 03/13/2015 BUT WAS NOT NOTIFIED TO THE CUSTOMERS. WE HAVE CONTACTED TOYOTA COMPANY, COMPLAINED ABOUT THIS INCIDENT, INFORMED THE RECALL AND WE ARE WAITING FOR A RESPONSE FROM TOYOTA AFTER A THOROUGH INVESTIGATION, COMPUTER DATA ANALYSIS FROM OUR RAV4. I AM HEREWITH ATTACHING THE DOCUMENTATION FOR THE RECALL. I AM VERY MUCH WORRIED & CONCERNED ABOUT THIS INCIDENT, SINCE IT IS OF VERY SERIOUS SAFETY ISSUE. I GOT OUT OF MY CAR AND FORGOT TO PUT IT IN PARK. BECAUSE THE CAR HAS KEYLESS START, I WAS ABLE TO LEAVE THE CAR WITHOUT TAKING A KEY OUT. THE CAR STARTED TO ROLL BACK AND BECAUSE I LIVE AT THE TOP OF A HILL, IT BEGAN TO PICK UP SPEED. MY NEIGHBOR LIVES AT THE BOTTOM OF MY DRIVEWAY AND THE CAR WAS HEADING STRAIGHT TO THEIR HOUSE. LUKILY, THE CAR HIT A TREE WHICH STOPPED IT FROM HITTING THEIR HOUSE AND POTENTIALLY KILLING SOMEONE. THE ADJUSTER JUST TOLD ME MY CAR IS TOTALED DUE TO THE SPEED AT WHICH IT HIT THE TREE. HOWEVER, HE ALSO INDICATED THAT THE SPEED AT WHICH THE CAR WAS TRAVELLING WHEN IT HIT THE TREE, IT MAY HAVE HIT THE NEIGHBOR'S HOUSE AND CONTINUED THROUGH THE OTHER SIDE. THIS IS A SIGNIFICANT SAFETY FLAW OF WHICH NO ONE I SPEAK TO ABOUT IT IS AWARE. THEY ACTUALLY TEST IT WHICH THEIR OWN CAR AND NOTICE THE SAME ISSUE. IN MY OPINION, THERE SHOULD BE A MODIFICATION TO THIS FEATURE TO PREVENT THIS ISSUE. ESPECIALLY IS THERE ARE KIDS NEAR THE CAR.

POTENTIALLY LETHAL TRANSMISSION FAILURE IN TOYOTA RAV4 IN MARCH 2013 I BOUGHT A NEW TOYOTA RAV4 AT ACTON TOYOTA, IN MASSACHUSETTS. ON FEBRUARY 21ST 2014, I WAS DRIVING THE VEHICLE IN SUDBURY, MASSACHUSETTS ON ROUTE 20 WHEN SUDDENLY IT WENT COMPLETELY HAYWIRE. THE ENGINE STALLED OUT ? RESTARTED ? STALLED OUT AGAIN ? RESTARTED. THIS WAS ALL WHILE THE VEHICLE WAS MOVING. I DROVE INTO THE LOCAL CITIZENS BANK CAR PARK, WHERE I PARKED HEAD IN TO A VERY LARGE SNOW DRIFT. I THEN DISCOVERED THAT ALL THE GEARS WERE GONE EXCEPT FOR 'D'! OI CYCLED SEVERAL TIMES THROUGH CLOSING THE CAR DOWN AND RESTARTING IT - NO CHANGE. OI COULD NOT PUSH THE CAR BACKWARDS AWAY FROM THE SNOW DRIFT ON MY OWN. OEVENTUALLY I WAS TOWED TO THE DEALERSHIP. OON ARRIVAL AT ACTON TOYOTA, THE SERVICE DIRECTOR - RANDY IRWIN - TESTED THE CAR AND IMMEDIATELY ENCOUNTERED THE ISSUE - SHIFTER IN REVERSE - REVVING THE ENGINE - NOTHING. HE TRIED THIS WITH ALL THE DIFFERENT SHIFTER POSITIONS OTHER THAN 'D', ALWAYS WITH THE SAME RESULT. HE THEN ENGAGED 'D' AND MOVED THE CAR FORWARD SLIGHTLY. HE THEN SLOWLY ENGAGED REVERSE AND THERE WAS A HUGE CLUNK FROM UNDER THE CAR. AND NOW REVERSE WAS ENGAGED! THE DEALERSHIP GAVE ME A LOANER VEHICLE AND KEPT THE RAV4. ON MARCH 3RD 2014, I PHONED IRWIN - HE SAID A TOYOTA REPRESENTATIVE DROVE IT THE PREVIOUS WEEK. THERE HAD BEEN NO RECURRENCE OF THE PROBLEM - NOW THEY WERE DOING A 'VISUAL INSPECTION' (WHATEVER THAT IS!) SINCE NOTHING CAME UP IN DIAGNOSTIC TESTS. ON MARCH 5TH 2014 I PHONED IRWIN AGAIN ? HE SAID THAT TOYOTA HAD TAKEN THE CASE OVER. TOYOTA THEN CALLED - THEY SAID THEY HAD NOT BEEN ABLE TO GET THE ISSUE TO RECUR, AND SO THEY REQUESTED THAT I PICK UP THE VEHICLE AT THE DEALERSHIP. THIS I DID. AT NO POINT DID TOYOTA EVER ACKNOWLEDGE A PROBLEM ? THEY ALWAYS STATED SIMPLY THAT THEY HAD NOT BEEN ABLE TO GET THE ISSUE TO RECUR. *TR

I PARKED MY CAR IN RESIDENTIAL NEIGHBORHOOD AND PROCEEDED TO CROSS STREET WITH MY CAR PARKED BEHIND ME. THANKFULLY A MAN FACING CAR YELLED "YOUR CAR!" MY NEW CAR WAS ROLLING DOWN THE STREET BACKWARDS, GAINING SPEED DOWN THE INCLINE, HEADED FOR CURB, SIDEWALK AND HUGE TREE. I CHASED THE CAR DOWN THE STREET, GOT DOOR OPEN, JUMPED INSIDE VEHICLE AS IT WAS MOVING AND SLAMMED ON BRAKE, INJURING MY BACK AND SHOULDER IN THE PROCESS. THE PUSH-BUTTON START ALLOWED CAR TO "FREE-WHEEL" OUT OF CONTROL WHEN TURNED OFF, LEFT IN DRIVE, AND KEY LEFT IN CAR. I AM IN AND OUT OF CAR MANY TIMES A DAY. NO OTHER CAR I HAVE EVER OWNED WILL ALLOW ME, WITHOUT WARNING, TO LEAVE CAR IF LEFT IN "DRIVE". I WAS NOT INFORMED BY TOYOTA THAT PUSH-BUTTON IGNITION WOULD ALLOW MY CAR TO BECOME A POTENTIAL WEAPON, FREE WHEELING, IF LEFT IN DRIVE AND UNATTENDED. I WOULD LIKE THIS SITUATION TO BE REMEDIED TO SAVE LIVES AND PROPERTY DAMAGE. EVEN AFTER GOING THROUGH THIS FRIGHTENING EXPERIENCE, I CANNOT BE SURE IT WON'T HAPPEN AGAIN WITHOUT AN EFFECTIVE WARNING SYSTEM. *TR
