EMANATE FROM THE VICINITY UNDER AND IN FRONT OF THE DRIVER SEAT AND ARE READILY DISCERNABLE BY BOTH DRIVER AND PASSENGERS. BRAKE CLICKING NOISES: OCCUR WHENEVER APPLYING LIGHT BRAKE PRESSURE. CUSTOMER HAS REPLICATED BRAKE CLICKING WHILE TEST DRIVING IDENTICAL RAV4 HYBRIDS.

WHILE IN REVERSE WITH FOOT ON BRAKE (STOPPED) THE VEHICLE WAS REAR ENDED. WHILE FOOT STILL ON BRAKE THE VEHICLE HAD UNCOMMANDED FORWARD ACCELERATION (ENGINE REVVED UP LIKE FOOT FLOORED THE GAS PEDAL). VEHICLE ACCELERATED FORWARD ~25 FT AND HIT THE FRONT OF ANOTHER VEHICLE (FOOT ON BRAKE PEDAL ENTIRE TIME). UPON IMPACT, THE VEHICLE HAD UNCOMMANDED ACCELERATION IN REVERSE (FOOT ON BRAKE PEDAL STILL). WAS ABLE TO TURN OF IGNITION AND PULL THE EMERGENCY BRAKE TO STOP THE VEHICLE AFTER MOVING ~25FT IN REVERSE. AT FINAL STOP, THE VEHICLE'S TRANSMISSION LEVER WAS IN THE DRIVE POSITION. NOTE: TOW TRUCK DRIVER STARTED VEHICLE TO GO IN REVERSE A FEW FEET FOR POSITIONING. WITH ENGINE RUNNING, VEHICLE WAS PUT IN REVERSE AND GAS PEDAL PRESSED GENTLY. VEHICLE WENT IN REVERSE (< 2MPH) AS IT SHOULD. FOOT APPLIED TO BRAKE AND VEHICLE PUT IN PARK IN ONE MOTION. VEHICLE IN PARK AND FOOT ON BRAKE THE ENGINE BEGAN TO REV UP AGAIN. KEY WAS TURN OFF. VEHICLE TOWED.

WAS AT DRIVE THROUGH CASH MACHINE, STOPPED, PUT INTO PARK, DID MY TRANSACTION, PUT CAR INTO DRIVE LEFT TO GET TO STREET VIA A SIDE STREET. NOTED AT STOPS THAT THE BRAKES WERE HARD AS WHEN ABS IS NOT QUITE RIGHT. ON A TOWN STREET, TWO PEDESTRIANS STEPPED OUT INTO MY LANE OF TRAVEL. I PUT ON MY BRAKES, HARD. THE ENGINE REV'D AND CONTINUED FASTER AND I KEPT MY FOOT ON THE BRAKE, THE WOMEN GOT OUT OF THE LANE. I REALLY DON'T RECALL SEQUENCE, I WAS TERRIFIED (HAVE HAD A CAR RUN AWAY WITH ME LONG AGO). BUT I DO KNOW IT WAS REV'ING HARD, I WAS SO TERRIFIED I DID NOT NOTE IF ANY DASH LIGHTS WERE ON, OR WHAT THE TACH (?) WAS DOING. RAV WAS IN DRIVE, I PUSHED LEVER TO N AND FINALLY TO PARK AND THE ENGINE WAS SHUT DOWN. I HAD PULLED TO THE SIDE OF THE STREET, SO FIRED IT UP, AND IT WAS 'NORMAL' BRAKES NO LONGER HARD. I CAUTIOUSLY PULLED OUT INTO THE LANE AND MADE IT TO THE TOYOTA DEALER. IT TRIED ONE MORE TIME TO DO A REV WHEN I PUT THE BRAKES ON, BUT NOT LIKE THAT FIRST TIME AS I IMMEDIATELY TURNED IT OFF AND PUT INTO NEUTRAL/PARK. TOYOTA SERVICE PERSON SHOWED CONCERN AND DROVE THE RAV AROUND FOR ABOUT 10 MINUTES AND COULD NOT GET IT TO REPEAT THIS. HE THINKS MAYBE I SHOULD HAVE THE SHOP BYPASS THE AUTOMATIC STOP ENGINE THING, PERMANENTLY BUT THERE'S NOT LONGER A WAY TO DO IN NEWER RAVS. SO PUSHED THE A BUTTON TO TURN IT OFF AND FEAR DRIVING IT. SERVICE MAN WAS GOING TO CALL ME BACK IF HE COMES UP WITH ANYTHING. HE MAY NOT RECALL THE UNINTENTIONAL ENGINE REVS OF A FEW YEARS AGO IN TOYOTAS, WHICH CAUSED ACCIDENTS AND DEATHS. IT WAS NOT THE FLOOR MATS I ONLY HAVE THE RUBBER ONES, 'BOLTED DOWN". FIRST FEW WEEKS AFTER PURCHASE RAV MADE BEARING-LIKE GRINDING NOISES WHEN THE AUTO STOP WAS TURNED OFF SO TURNED IT BACK ON. WHEN A ON OR OFF, VEHICLE JERKS COMING TO A STOP AND WHEN MOVING FORWARD AFTER A STOP. CAR HAS <4000 MILES. BOUGHT NEW. *TR

TL* THE CONTACT OWNS A 2019 TOYOTA RAV4. WHILE DRIVING APPROXIMATELY 2 MPH AND ATTEMPTING TO PARK, THE VEHICLE DID NOT STOP. THE BRAKE PEDAL WAS DEPRESSED AT THE TIME OF THE FAILURE. THE VEHICLE CONTINUED TO MOVE, DROVE OVER A CURB AND SIDEWALK, AND CRASHED INTO A STOREFRONT. POLICE REPORT NUMBER: [XXX] WAS FILED. THERE WERE NO INJURIES. THE FRONTAL AIR BAGS DID NOT DEPLOY. THE VEHICLE WAS TOWED TO A BODY SHOP. THE VEHICLE WAS NOT TAKEN TO A DEALER FOR DIAGNOSTIC. TESTING OR REPAIRS. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE AND STATED THAT THEY WOULD SEND A FIELD MECHANIC TO DIAGNOSE THE VEHICLE. ELECTRONIC PARKING BRAKE MALFUNCTION. THE VEHICLE HAD NOT BEEN DRIVEN IN A FEW DAYS, IT WAS PARKED IN OUR ENCLOSED GARAGE. STARTED VEHICLE SHIFTED INTO DRIVE, LET OF BRAKE AND VEHICLE WOULD NOT MOVE FORWARD. I THEN APPLIED LIGHT THROTTLE AND STILL THE VEHICLE WOULD NOT MOVE. I COULD FEEL RESISTANCE FROM THE REAR WHEELS. I SHIFTED BACK INTO PARK AND MANUALLY CYCLED THE PARKING BRAKE SWITCH. NOTICED THAT THE PARKING BRAKE LIGHT INDICATOR IN THE INSTRUMENT CLUSTER WOULD NOT ILLUMINATE. NO AUDIBLE NOISE OF PARKING BRAKE ACTUATOR FUNCTIONING. CYCLING THE PARKING BRAKE SWITCH DID NOTHING. I TRIED TURNING THE ENGINE OFF AND BACK ON, THIS DID NOT RESOLVE THE SITUATION. SHIFTED BACK INTO DRIVE AND GENTLY APPLIED LIGHT THROTTLE, THEN SHIFTED INTO REVERS AND APPLIED VERY LIGHT THROTTLE. I REPEATED THIS A FEW TIMES AND WITH A SUDDEN LOUD CLUNK THE E- BRAKE RELEASED AND THE VEHICLE MOVED FORWARD. I THEN CYCLED THE E-BRAKE SWITCH A FEW TIMES AND IT APPEARS TO FUNCTION PROPERLY AGAIN. THROUGH OUT THIS NO MESSAGES OR WARNING LIGHTS WERE DISPLAYED INDICATING ANY PROBLEM WITH THE E-BRAKE OR ANY SYSTEM. THIS VEHICLE HAS 2,400 MILES ON IT. IT HAS NEVER BEEN SUBJECTED TO ANY ENVIRONMENTAL CONTAMINATES THAT COULD POSSIBLY CAUSE THIS ISSUE. IT HAS NEVER BEEN OFF-ROAD, NO EXPOSURE TO SNOW, MUD, DIRT OR FREEZING TEMPS. THIS IS A SAFETY HAZARD! I HAVE NO IDEA IF OR WHEN THE ELECTRONIC EMERGENCY BRAKE MAY FAIL TO ENGAGE OR DISENGAGE. I TALKED TO THE DEALER AND THERE SUGGESTION WAS TO WAIT UNTIL IT HAPPENS AGAIN AND HAVE IT TOWED IN. THIS IS AN ALL WHEEL DRIVE VEHICLE SO ATTEMPTING TO WINCH IT UP ON A TOW TRUCK WOULD LIKELY FREE THE STUCK BRAKE, OR DAMAGE THE DRIVE TRAIN. THIS IS EXTREMELY FRUSTRATING.

I STARTED IT AND NOTICED IMMEDIATELY SOMETHING WAS WRONG. VEHICLE STARTED MOVING AND I TRIED TO APPLY BRAKE. BRAKE WAS HARD INITIALLY AND VEHICLE CONTINUED MOVING. THEN I WAS ABLE TO DEPRESS THE BRAKE PEDAL ALL THE WAY BUT IT KEPT MOVING. WITH MY DAUGHTER IN THE CAR I GOT PANIC. I SWITCHED TO P AND VEHICLE KEPT GOING, THOUGH SLOWLY ON THE STREET. THEN I TURNED POWER OFF AND REMOVED THE KEY. AT THIS POINT ALL DASH BOARDS LIGHTS WERE ON AND IT WAS MAKING A LOUD, WIRED NOISE. IT LASTED SEVERAL SECONDS AND VEHICLE KEPT MOVING DURING THIS TIME. FINALLY IT CAME TO A STOP IN THE MIDDLE OF THE STREET. THE VEHICLE HAS ONLY 4200 MILES. THE SCARY THING IS, BRAKE FAILED FOR NO REASON AND ALL DASH BOARD LIGHTS WERE ON EVEN AFTER THE KEY WAS REMOVED.

PURCHASED MY NEW RAV4 ON 12/2017. ON 8/13/2017 I WAS AT A DRIVE THROUGH AND STOPPED AT THE END OF THE LINE AND THE SERVICE BRAKE WHEN I PRESSED THE PEDDLE THE BRAKE WAS FIRM AND STOPPED CORRECTLY. THE LINE MOVED AND I LIFTED MY FOOT OFF OF THE SERVICE BRAKE AND THE LINE STOPPED AND I APPLIED THE SERVICE BRAKE PEDDLE AND HAD TO PRESS THE PEDDLE FURTHER THAN I PRESSED THE PEDDLE THE FIRST TIME I STOPPED IN THE LINE. WHILE STOPPED IN THE LINE THE SECOND TIME THE RAV4 STARTED TO ROLL FORWARD AND I HAD TO PRESS THE SERVICE BRAKE EVEN FURTHER TO GET THE RAV4 STOP. THE LINE MOVED FORWARD AND I LIFTED MY FOOT OFF OF THE SERVICE BRAKE PEDDLE AND THE LINE STOPPED AND WHEN I PRESSED THE SERVICE BRAKE PEDDLE THE SERVICE BRAKE PEDDLE WENT TO THE FLOOR AND DID NOT SLOW UNTIL I STRUCK THE VEHICLE IN FRONT OF ME. AFTER THE ACCIDENT MY SERVICE BRAKE PEDDLE WAS VERY SPONGY AND SLOW TO RESPOND WHEN PRESSING THE SERVICE BRAKE PEDDLE. I TOOK MY RAV4 ON 8/14/2017 TO LOCAL DEALERSHIP TO HAVE MY SERVICE BRAKES CHECKED. WHEN I TOOK THE RAV4 IN I DID NOT HAVE ANY PAPER WORK TO SIGN. THE AFTERNOON THE SERVICE ADVISOR SAID MY RAV4 WAS READY. WHEN I WENT TO GET MY RAV4 THE SERVICE ADVISOR SAID NO CODES AND DID NOT GIVE ME ANY PAPER WORK AND I DID NOT SIGN ANY PAPER WORK. WHEN I GOT IN TO DRIVE THE RAV4 AWAY MY SERVICE BRAKE PEDDLE WAS FIRM NOT SPONGY AND RESPONDED QUICKLY AS VERY LITTLE MOVEMENT TO PRESS THE SERVICE BRAKE PEDDLE TO STOP THE RAV4. MY SERVICE ON THE TOYOTA WEB SITE SERVICE HAS NEVER REPORTED THE SERVICE EVEN TODAY. I CALLED THE TOYOTA AND TOLD THEM OF THIS SITUATION AND THE TOYOTA REPRESENTATIVE TOLD ME TO BRING MY RAV4 BACK TO THE DEALERSHIP ON 8/24/2017 AND HE WOULD DOWNLOAD THE EVENT RECORDER AND DID ON 8/31/2017. NO ISSUES WAS REPORTED ON THE EVENT RECORDER. LETTER SHOWED NO REFERENCE THAT I HAD MY SERVICE BRAKES CHECKED ON 8/14/2017. SERVICE BRAKE IS SPONGY WHILE STOPPED NOW.

THE VEHICLE WAS TAKEN TO A LOCAL DEALER (PERFORMANCE TOYOTA LOCATED AT 5676 DIXIE HWY FAIRFIELD, OH 45014) WHERE IT DIAGNOSED THAT THE BRAKES MET TOYOTA SPECIFICATIONS. THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE AND DID NOT ASSIST. THE FAILURE MILEAGE WAS 1,400. UPDATED 11/02/17*LJ CONSUMER STATED THE DISTANCE THE BRAKE PEDAL TRAVELS BEFORE THE BRAKES ARE APPLIED TO STOP THE CAR. DISTANCE IS INCREASING AS WE PUT MILES ON THE VEHICLE. THE BRAKE PEDAL WILL CONTINUE MOVING TOWARDS THE FLOOR AFTER STOPPING. AS IN A DRIVE THRU LINE. BRAKE PEDAL IS SOFT AND SPONGEY. MIMCS AIR IN BRAKE LINE. UPDATED 10/4/18*JB

AS I WAS APPROACHING THE MAIL BOX AREA IN MY APARTMENT COMPLEX AT APPROXIMATELY 15 MPH, I PUT MY FOOT ON THE BRAKE TO TURN INTO THE PARKING AREA IN FRONT OF THE FOUR METAL MAILBOXES AND PUT THE VEHICLE IN "PARK" WHEN THE VEHICLE SUDDENLY LURCHED FORWARD AND JUMPED THE SIX-INCH, BRICK-PAVER CURB. I CRASHED INTO THE MAILBOX STRUCTURE, COMPLETELY UPLIFTING THE BASE THAT WAS BOLTED INTO THE CONCRETE AND HIT A WOOD TRASH CONTAINER HOLDER. I DEPRESSED THE BRAKE PEDAL HARDER BUT MY VEHICLE WOULD NOT SLOW DOWN OR STOP AND I ALSO COULD NOT CONTROL THE STEERING. I CONTINUED UP A HILL (AT 45-50 MPH) CONSISTING OF TALL GRASS, WILDFLOWERS, ROCKS, ETC., ACROSS A WALKING PATH OF WOODCHIPS AND MULCH, THEN DOWN THE HILL (STILL WITH MY FOOT ON THE BRAKE) GRAZING THE BRANCHES AND LEAVES OF A SMALL TREE BEFORE COMING OFF THE SIX-INCH CURB AND BACK ONTO THE STREET, WHERE I HIT A LIGHT POST. I MANAGED TO LIFT THE PARKING BRAKE, SLAM ON THE BRAKE AGAIN AND THE VEHICLE ROLLED TO A STOP. IT SHOULD BE NOTED THAT NONE OF THE AIRBAGS DEPLOYED.
